To attack the wearer, the monster must succeed on a DC 12 Will saving throw. If the monster passes the check, the player is attacked normally–otherwise, the monster will chose another target. Similar to the sanctuary spell, the effect only works if there is more than one party member left. If the wearer is the only character left alive the monster will attack normally. In addition, if the wearer performs a hostile action (attacks, casts a damage-inflicting spell, or casts a spell requiring the monster to make a saving throw) against the monster, all effects of the Hat are negated for the rest of that combat.
Once the monster succeeds on the saving throw, it is free to attack that character for the remainder of combat. I.e., it doesn’t have to roll a saving throw every round.
